Financial Policy
We accept many forms of insurance. Patients should inquire about our participating insurance plans prior to making an appointment. If you are a member of a health maintenance organization (HMO) that requires a referral from your primary care physician, please bring that referral form with you. Any fees or co-payments are expected to be paid at the time of service.
